This booklet would be provided to the judges of the 2012 National Showchoir Championship. This was used in season 3 episode 21 of Glee titled “Nationals.” It lists several “Special Recognition” options and a rating system. It is unclear as to whether or not this booklet was filmed during this episode. If it was it is believed to have been a stagnant prop or scenery item.
